Explanation of Benefits (EOB) Statement – Our customer utilizes this Explanation of Benefits (EOB) statement to summarize information for their customers about the health care services they have received, including costs paid (covered) and costs not paid (not covered). The number of services listed on this statement varies from customer to customer. Azuba CCM enables the definition and composition of repeating rows. This enables virtually an unlimited number of services (rows of data) from our customer’s database to be merged into the composed document.

Premiums Paid Confirmation Letter – Our customer utilizes this letter to notify their customers that they have paid their annual premium for their life insurance policy. Azuba CCM enables the creation and support of multi-language documents, based on each customer’s individual preference – in this case, English or Spanish.

Unemployment Claim Filing Notice – Our state government customer utilizes this notice to inform the most recent employer of an individual who has filed for unemployment benefits. Attached with this notice is a form to protest or appeal the determination. Azuba CCM enables the inclusion of enclosures or attachments according to the template design.
